- To enroll in a portal for your doctor’s office visits, request an application form when you visit your Ascension Medical Group doctor’s office.
- Setting up your personal patient portal is quick and easy, requiring just a few minutes and four simple steps. Accessible 24 hours a day, you can use it from your computer, tablet, or mobile device at any time!
- Establish a username and password, ensuring both are unique and robust to enhance the security of your patient portal account. Adhere to the provided guidelines for crafting a strong password.
- Consent to the terms and conditions by reviewing them carefully. Once you are comfortable with the terms, mark the box to signify your agreement.
- Select “Sign Up.” After filling out the registration form, click the sign-up button to successfully create your account.
